2021 Hockey Camp

Please complete and submit this form online on or before the first day of camp.

If you have any questions regarding payment, please contact the Finance Office at (216) 651-0222 ext. 328.

Both the registration form and camp fee are due on or before the first day of camp in order to complete the registration process and secure your child's spot in the camp.

This form should take approximately 5 minutes to complete. You cannot save and return to the form once you have started to complete it, so please be prepared to complete the form and credit card payment in one sitting.

Please be aware that completing this form on a mobile phone may cause duplicate entries and payments. Please complete the form on a desktop browser or tablet if available.

                  4-4:45 p.m. (Boys/Girls Grades K-3)
                  5-5:45 p.m. (Boys/Girls Grades 4-5)
                  6-6:45 p.m. (Boys/Girls Grades 6-7)
                  7-7:45 p.m. (Boys/Girls Grade 8)

Youth Hockey Camp - Each Wednesday from June 16 to June 30

Date: June 16, June 23 and June 30
Times: 
4-4:45 p.m. for grades K-3 Boys and Girls
5-5:45 p.m. for grades 4-5 Boys and Girls
6-6:45 p.m. for grades 6-7 Boys and Girls
7-7:45 p.m. for grade 8 Boys and Girls
Where: Brooklyn Rec Center 
Cost: $75 skater / $50 goalie
